Emergency Permission Form
This form is to be completed by a student’s parent or guardian, is required for entry to school for ALL students regardless of grade. This form is required annually. Please indicate your child’s “rising” grade on the form.

Child and Adolescent Health Examination Form
This form is to be completed by your medical practitioner is required for entry to school. Please read below:

• All new students
• All preschool and kindergarten students
• All returning students in grades 1
• All 6^th through 12^th grade students who plan to play sports

Interval Health History Form
This form is required for all students in grades 7 through 12 who wish to participate in team sports. If the student has had a physical exam WITHIN 30 days prior to the start of the season, they do not need an Interval Form.
