Child and Adolescent Health Examination Form
This form is to be completed by your medical practitioner and is required for school entry for the following grades:
As soon as your child has their next annual well-visit, please upload a copy to the August health portal. Additionally, all preschool students are required to have an annual flu vaccine by December 15 each year.
Interval Health History Form
This form is required for all students in grades 7 through 12 who wish to participate in team sports. If the student has had a physical exam WITHIN 30 days prior to the start of the season, they do not need an Interval Form.
